Primary Care Partners (PCPs)

What is a Primary Care Partner?

Primary Care and Physician Partners include inter-professional primary care providers (physicians, nurse practitioners, physician assistants) and specialists. These partners either work within, or provide care to, diverse patient populations within the Ottawa West Four Rivers OHT. Members are systems-minded and want to work collaboratively with our primary care, communities  and the 50 plus partner organizations connected to the Ottawa West Four Rivers OHT to influence the system changes that are needed.
