Amy Boudreau, BScN, MHSA is Carefor's Director of Integration and Sustainability, in this role she leads Carefor's transition into the Ontario Health Teams.
Amy joined Carefor's leadership team in 2017 following a long-term post as Director, Strategic Planning and Operations for the Ontario Centre of Excellence for Child and Youth Mental Health at CHEO. Prior to this, Amy spent time developing national health care standards at Accreditation Canada and developed a pediatric home care program and led other strategic initiatives for the VON-Ottawa-Carleton Branch (now Carefor). Before this, Amy spent time pursuing a nursing career in pediatric acute-care facilities, holding positions at both SickKids in Toronto and at the IWK Health Centre in Halifax. As an active member of the Canadian College of Health leaders, Amy is deeply committed to lifelong learning as a health leader and strives to coach others to learn, evolve and grow to the best of their abilities. Most notably, Amy is a proud mother to three incredibly active, forever fun and amazingly kind boys.

Jennifer Lalonde is Executive Director of Ottawa West Community Support, a multi-service seniors support agency.
Ms. Lalonde has over 20 years of professional and management experience. Prior to arriving at OWCS in 2016, Ms. Lalonde was a Director at the Canadian Bar Association. Here she oversaw the activities of numerous professional legal groups and was a key member of the management team responsible for the strategic and operational planning for the association. Ms. Lalonde has strong ties to her community and believes in the value of giving back through volunteer involvement. Over the years, she has volunteered with many not-for-profit and charity organizations including the Girl Guides of Canada, Amethyst Women’s Addiction Centre, and most recently the Community Navigation of Eastern Ontario. Ms. Lalonde has an MBA from Carleton University. She is married with two children. |
About the Action Team:The Complex Chronic Diseases (CCD) Action Team is tasked with understanding the current state of health care within our OHT for those who experience complex chronic disease. Working to improve the care for those living with CCD, our action team will utilize a population health management approach to guide us in working through each of the phases of design, implementation and evaluation of this work. Diabetes was selected as the initial population focus for the action team and subsequent efforts beyond diabetes will span to include additional chronic diseases. Our current goal is to ensure a people-centered approach with a focus on diabetes, where we would ensure our interventions takes a holistic approach to either preventing and/or supporting people who are living with diabetes in our OHT.

Actively Learning: We were pleased to host 2 in-person sessions in the month of November, during which we were fortunate to hear from both lived experience partners and staff working in diabetes care provision. These forums afforded an opportunity to discuss the current state of diabetes care in our OHT – what is working well, as well as barriers that currently exist. Those gathered then also participated in discussion to share creative means of lessening barriers, improving our care system and supporting those living with diabetes and their families. The wisdom and creative thought that was displayed in those sessions will be extremely valuable as we move forward with developing change ideas and we are grateful to those who made time to participate.
